Diamond Counseling Center

For Professionals

Information for Referring Professionals

This page is for clinicians, discharge planners, attorneys, probation officers, EAP coordinators, and anyone else placing a client in outpatient substance use treatment in Middlesex County.

Everything you need to make a referral decision is on this page. If something's missing, call (732) 248-1805 and ask.

What we are

A licensed New Jersey outpatient substance use treatment program in North Brunswick, offering intensive outpatient and outpatient levels of care to adults, with integrated treatment for co-occurring mental health conditions.

Who we accept

Adults 18 and over who are:

  • • Medically stable and not in need of medically supervised withdrawal
  • • Appropriate for an outpatient level of care
  • • Able to attend a scheduled group program
  • • Presenting with substance use, with or without a co-occurring mental health condition

Who we are not the right placement for

Stated plainly, because you need the exclusion criteria more than you need our brochure:

  • • Anyone requiring medically supervised detoxification — we do not provide detox
  • • Anyone requiring 24-hour care, residential treatment, or partial hospitalization — we do not provide those levels of care
  • • Anyone in acute psychiatric crisis, or actively suicidal, requiring a higher level of stabilization
  • • Anyone under 18
  • • Anyone whose clinical presentation cannot be safely managed in a group outpatient setting

If a client needs detox first, we're a good step-down destination afterward. Call us while they're still admitted and we'll coordinate the handoff.

Diamond does offer licensed adult substance use Partial Care, which is not partial hospitalization.

Levels of care and actual schedules

Intensive Outpatient (IOP)

IOP meets three times per week for three hours per session and is typically 12 weeks, with length based on clinical need. Groups generally include 6 to 10 participants and may include up to 12.

Day IOP: Monday–Thursday, 9:30 AM–12:30 PM (Mon & Wed in person, Tue & Thu virtual)

Evening IOP: Monday–Wednesday, 6:00–9:00 PM (Mon in person, Tue & Wed virtual)

Outpatient (OP)

Lower intensity. Used as a step down from IOP, or as an initial placement when IOP exceeds clinical need.

Also provided

Individual therapy, family therapy, telehealth, relapse prevention planning, life skills. Psychiatric evaluation and medication management are available to Diamond clients through our collaborating psychiatric nurse practitioner. Diamond Counseling Center does not provide medication assisted treatment or medication services itself; when medication is clinically indicated, we coordinate a referral and share information only with your written consent. We also coordinate with a client's existing prescriber throughout treatment.

Substance use evaluations

We perform evaluations for courts, probation and parole, DUI and IDRC referrals, DCPP, and employers. Reports include a clinical summary, diagnostic impression where warranted, and a level-of-care recommendation, released to the requesting party under a signed 42 CFR Part 2 consent.

Turnaround: Evaluation appointments are generally available within 1 to 3 business days depending on timing and availability. Completed written evaluation reports are usually provided the same day or next business day. Admissions are generally available within one week.

Evaluations are billed to insurance.

Insurance

In network with Aetna, UnitedHealthcare, and Horizon Blue Cross Blue Shield.

Most NJ Medicaid plans accepted.

For non-contracted commercial plans we pursue single-case agreements, and self-pay is available.

We verify benefits and handle prior authorization ourselves, so your client isn't sent away to make phone calls — a step where referrals are frequently lost.

How fast a client can start

Our process is predictable, which matters more to you than a fast number we can't hit.

1

Evaluation, typically within three business days of your call. About an hour with a licensed clinician.

2

Acceptance into the program, based on that evaluation.

3

Evaluations within 3 business days; admissions within 1 week.

If you're working against a detox discharge date, call us before the client discharges — (732) 248-1805 — and we'll build the evaluation around that date rather than after it.

If we can't meet your timeline, we'll say so on the first call.

How to make a referral

Contact our Intake Department directly:

(732) 248-1805

Fax: (732) 720-8323

info@diamondcounselingcenter.net

Send whatever you have. A phone call with the basics is enough to start — we'll tell you on that call whether the client is appropriate for our level of care rather than making you wait for a callback to find out.

Documentation and continuity of care

What we need from you

  • • A signed release
  • • The reason for referral
  • • Relevant clinical history
  • • Current medications and prescriber
  • • Any discharge summary if the client is stepping down from a higher level of care

What we provide back, with a signed release

  • • Confirmation of admission
  • • Attendance and compliance reporting where required
  • • Treatment plan and progress information
  • • A discharge summary with continuing care recommendations

On 42 CFR Part 2

Substance use disorder records carry federal protections stricter than HIPAA. We cannot acknowledge that a client is enrolled here, or share any information, without a valid written consent that names you specifically. If you're referring, send the release with the referral — it prevents a delay later. If you need our consent form, ask and we'll send it.
