IOP vs PHP: Understanding Levels of Care
Choosing the right level of addiction treatment for your situation.
When seeking addiction treatment, you'll often hear about IOP and PHP. Both are outpatient programs — meaning you live at home — but they differ significantly in intensity, structure, and who they're best for.
This guide explains both levels of care, compares them side-by-side, and helps you understand which might be right for your situation.
What is IOP (Intensive Outpatient Program)?
IOP is structured addiction treatment that fits around your life. You attend sessions several times per week while continuing to work, care for family, and sleep at home.
Time Commitment
9–20 hours per week
Typically 3 hours/day, 3 days/week
Duration
Typically 12 weeks
Adjusted based on progress
IOP Includes:
- Group therapy (6–10 people)
- Individual counseling sessions
- Education on addiction, triggers, and coping skills
- Psychiatric services are available through our collaborating psychiatric APN
- Drug screening
What is PHP (Partial Hospitalization Program)?
PHP is intensive day treatment — almost like inpatient care, but you go home at night. It's designed for people who need more structure than IOP provides but don't require 24-hour supervision.
Time Commitment
20–30 hours per week
Typically 5–6 hours/day, 5 days/week
Duration
2–4 weeks
Often steps down to IOP
PHP Includes:
- Multiple group therapy sessions daily
- Individual therapy multiple times per week
- Intensive psychiatric oversight
- Daily structure and accountability
- Meals often provided

PHP Structure
- • Full days (5–6 hours), Monday–Friday
- • Structured daily schedule
- • Multiple therapy sessions per day
- • Meals and breaks built in
- • Daily check-ins and accountability
- • Often steps down to IOP after 2–4 weeks
Who is Each Level Best For?
IOP is Best For:
- People with moderate addiction who need structure but can maintain stability at home
- Working professionals who can't take weeks off
- Parents who need to be home for their children
- People stepping down from PHP or residential treatment
- Those who have completed detox and need ongoing treatment
- People who tried weekly therapy and need more intensity
PHP is Best For:
- People with severe addiction who need intensive daily support
- Those stepping down from inpatient or residential treatment
- People in crisis who need stabilization but not 24-hour care
- Those with co-occurring mental health conditions needing intensive psychiatric oversight
- People who have relapsed multiple times and need maximum structure
Cost Comparison
Cost depends on your insurance plan and the level of care you need. If you have insurance, we verify your exact benefits before your first visit at no cost. Call (732) 248-1805 and we'll tell you what your plan covers.
Why do costs differ? PHP involves significantly more treatment hours per week than IOP — typically 20–30 hours for PHP versus 9–20 hours for IOP.
Evening IOP: Treatment for Working Professionals
One of IOP's biggest advantages over PHP is flexibility. Evening IOP programs allow you to work during the day and attend treatment after hours.
Our Evening IOP Schedule
- Days: Monday, Tuesday, Wednesday
- Time: 6:00 PM – 9:00 PM
- Format: In-person (North Brunswick) or virtual
Evening IOP means you don't have to choose between your job and your recovery. You work your regular hours, then attend treatment in the evening. Your employer doesn't need to know, and your income stays intact.
The Treatment Continuum
IOP and PHP aren't competing options — they're part of a continuum of care. Many people move through multiple levels:
1
Detox (if needed)
Medical supervision for withdrawal — 3–7 days
2
Residential/Inpatient (if needed)
24-hour care — 28–90 days
3
PHP
Full-day treatment while living at home — 2–4 weeks
4
IOP
Structured treatment while working/living normally, typically 12 weeks
5
Outpatient
Weekly therapy and support — ongoing
Not everyone needs every level. Some people start at IOP; others step down from PHP. A clinical assessment determines the right starting point.
How to Decide: IOP or PHP?
Ask yourself these questions:
Can you maintain stability at home?
If yes → IOP may be sufficient. If you need daily structure to stay safe → consider PHP.
Do you need to keep working?
If yes → IOP (especially evening IOP) fits around work. PHP requires full days, making employment difficult.
Are you stepping down from inpatient or detox?
PHP often serves as a bridge. After PHP stabilizes you, step down to IOP for longer-term treatment.
How severe is your addiction?
Moderate addiction → IOP. Severe addiction or recent crisis → PHP may be needed first.
The Bottom Line
IOP is flexible, work-compatible treatment for people with moderate addiction or those stepping down from higher levels of care. It's 9–20 hours per week, typically over 12 weeks.
PHP is intensive day treatment for people who need maximum structure but can still live at home. It's 20–30 hours per week over 2–4 weeks.
The right level depends on your specific situation. A clinical evaluation can help determine the best starting point for your recovery.
